Passport Quick Tips
- ALL APPOINTMENTS ARE BASED ON AVAILABLE TIME SLOTS. NO EXCEPTIONS WILL BE MADE. Use the Online Appointment Scheduler to schedule an appointment. Appointments can be made up to 30 days in advance.
- Use black ink only on your application.
- Don't sign the application before you come - it must be signed in the presence of an agent.
- Bring a check or money order - Scottsdale cannot take credit/debit cards for passport processing.
- Bring all the necessary supporting documents for each person applying.
- For children under age 16, BOTH parents must appear with child/children or provide required documentation authorizing issuance of a passport. Please refer to Children Requirements under “Passport Links” for more details and options.
- Make sure your photo is the right size & meets the guidelines.
- The city of Scottsdale is a passport application acceptance facility: we forward your completed application to the U.S. Department of State, who issues the passports. You will not leave our office with your passport in hand.

New Applicants
- One official passport photo (2"x 2")
- Certified Birth Certificate or Expired Passport
- Driver's License or other ID (To verify what is acceptable go to the forms page)
Renewal Applicants
Instruction on how to renew an expired passport, you may be able to do this by mail.

Passport payment shall be made by CHECK OR MONEY ORDER ONLY, payable to U.S. Department of State. $35 execution fee may be made by check, cash, money order or credit card, payable to City of Scottsdale.
Passport Cards - The passport card will facilitate entry and expedite document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. It will otherwise carry the rights and privileges of the U.S. passport book and will be adjudicated to the exact same standards. Photographs
You need one (1) passport photo size 2x2 taken within the last six (6) months. It must be front view with a white or off-white background. (Photographs can be taken at CVS, Kinkos, Walgreens, etc.)
Evidence of Citizenship
ONE of the following documents MUST be presented:
- Certified copy of your birth certificate
- Certificate of Naturalization or of Citizenship
- Expired Passport
- Consular Report of Birth
- For child passport renewals, applicants under age 16 will be required to submit a certified birth certificate AND their previous passport.
- If you are unable to present an acceptable identification document, you may bring a person who has known you for more than two (2) years. To be acceptable, the witness must be either an American citizen or permanent resident alien and must present appropriate identification.
Appointments Only
Passport assistance and processing will be by appointment only. Please be sure to use our Passport Scheduler to reserve your appointment. Make sure that you have all the required documents and that you complete all forms. Bring required documents and forms to your appointment. When you arrive please see a cashier and advise them you are here for a passport appointment and provide the name that the appointment was scheduled under.
